Your 2025 Benefits Enrollment Kits are only available online and will not be mailed to your home. However, County employees without a County email will continue to receive an OE Kit to their home address. The Kit, which includes the Open Enrollment Guide, Document Certification Form and other flyers from our different vendors can be viewed here at EBC online 24/7. Just click on any of the links to your right. We encourage you to pay special attention to the 2025 Open Enrollment.

2025 Employee Benefits Handbook
In addition to this website, 2025 Employee Benefits Handbook is a tool that contains everything you want to know about the employee benefits offered at Alameda County. Click on the Employee Benefits Handbook link in the box on the right and save it to your desktop for a quick reference.
Please note that the handbook is updated periodically, make sure you have the latest version by checking back from time to time.
Deadline for the Document Certification Process
When you add a new dependent to your coverage or start/continue your election in the Share the Savings Program, you will need to submit a Document Certification Form, along with supporting documentation, by November 5, 2024, the last day of the Annual Open Enrollment window. No exceptions or extension to this deadline will be granted.
United Healthcare Select Plus PPO Plan
The UnitedHealthcare (UHC) Select Plus PPO Plan offers you a national network and out-of-network coverage. You can receive care from a provider in or out of the County’s plan network. However, it’s important to be aware that staying in the County’s plan network and using network doctors can help you save money.
You don’t need to choose a primary care provider (PCP) or obtain referrals to see a specialist. However, we recommend that you select a PCP to help you manage your care. A PCP can also help you avoid unnecessary tests and services, and can connect you to a network specialist, if needed.
United Healthcare Advantage Plan
The UHC Signature Value Advantage HMO plan designs and copays are the same as the $15 and $40 co-pay plans with the UHC Signature Value HMO plans. The one important difference is the network. The UHC Advantage network is UHC’s “high-performance network,” a smaller subset of the overall network selected for UHC’s high quality-care. For more information, please refer to the Employee Benefits Handbook.
Providing New York Life Insurance with “Evidence of Insurability (EOI) Made Easy”
Providing Evidence of Insurability (EOI) is simple. When you elect Supplemental Life for the first time or apply for additional coverage above your “guaranteed issue” you will receive access to an on-line EOI form. Access the on-line EOI form in eBenefits, complete and submit the EOI form electronically to New York Life Insurance.
New York Life will immediately send you an email confirming receipt of your completed EOI form. If you do not receive this email, New York Life Insurance did not receive your request. Please resubmit. Once New York Life Insurance confirms receipt of your EOI, you will receive a letter confirming approval or denial of your request for enrollment and/or your request for benefit amounts above guaranteed issue.
Designated Beneficiaries
Although you can update your Life Insurance beneficiaries anytime during the year, it is a good idea to use the Open Enrollment period to make sure your listed beneficiaries are up to date. You can change your beneficiary assignment by obtaining a New York Life Beneficiary Designation Form, under the “Forms” tab at EBC Online.
During Open Enrollment, you can designate your beneficiaries online through your eBenefits enrollment. However, if you are married and are designating more than 50% to someone other than your spouse, please print and complete a “Spousal Consent Form”.
Vacation Sellback Option to Purchase Disability
If you are eligible and you elected (in your current benefit plan year) the vacation sellback option to purchase Disability Insurance, you will need to again elect the vacation sellback option for each new plan year. Vacation sellback option to purchase Disability Insurance defaults to payroll deduction each year unless you re-enroll (elect Vacation Sellback option) to use vacation sellback to purchase Disability Insurance for the new benefit plan year. Keep in mind that you are automatically switched to payroll deductions if you do not have sufficient accrued vacation to pay for your Disability Insurance plans.

- Accident, Critical Illness, and Hospital Indemnity insurance coverage is provided through MetLife. Check out the plan information in the Voluntary benefits section for details on each benefit and to find out more information, please visit County of Alameda | MetLife.
- Group Legal Plan insurance is provided through MetLife for you to gain access to legal professionals at an affordable rate. The MetLife Plus Parent option covers you, your spouse and dependents, and also includes your parents and parents-in-law for estate planning, identity theft issues, document review and more. - MetLife Identity & Fraud Protection Plan provided by AURA helps safeguard your identity, money, assets, family, reputation, and privacy. It also requests the removal of your personal information from data broker lists to help reduce spam, robocalls, and more. Receive alerts to inquiries to your credit, suspicious transactions on your bank accounts and changes to your home or car titles.
